Environmental cancer risk factors are well documented with lots of data available from open source. But heterogeneity in data quality, granularity, exposure duration, unit of measure,.. may limit interoperability when multisource data pooling is needed. The project aims at defining universal standards for water and air risk factors.

There is a need for standardization of open-source data related to environmental risk factors for cancer (like what has been done for clinical and biological data in oncology through the OSIRIS project) in order to facilitate data collection, sharing and interpretation. The project outcome will be a structure for a minimal set of air and water agents with scientific evidence of carcinogenicity for at least one cancer, be it a solid tumor or a hematologic malignancy, including standard terminology to be used across institutions when dealing with such data.


The baseline from which we propose to start is an extract of all the agents considered as environmental factors that are listed by the IARC monographs, an internationally recognized reference. The list of agents will be sorted by classes of risk (I, IIA and IIB), medium (air, water), nature (chemicals, physical, biological) and associated cancers.


From the above-mentioned list, the project team will have to define the nature of the data (discrete or not, quality standard, acceptable age, unit of measure), the source (France at least), the duration of exposure, the minimum actionable geographical perimeter, as well as the possible ethical and regulatory constraints. We propose to keep the issue of OMOP and FHIR standards out of our project scope at this stage.


In terms of methodology, the project team can apply to environmental risk factors the work performed by OSIRIS on clinical and biological data, with their active support. As environmental data are geo-dependent, we propose to leverage the great experience of GEOCANCER in the field of geocoding.
